Dental Exams

School Dental Exams:

Pennsylvania requires students upon entrance to school (Kindergarten or first grade), third Grade, seventh grade and any student without a dental exam on file will need a completed dental form. Private dental exams are encouraged to be done by a family dentist up to one year before the entry into the specified grade. A private dental from can be printed, see below.
 
If desired, the school dentist can perform the needed dental exam. There is no charge for the school exam.  Parents need to notify the school nurse if they would like to have their child examined by the school dentist. Signed parental permission is required for this exam. A permission form can be printed, see below.  Parents/guardians are welcome to attend the school exam.  Contact the school nurse if you would like to be present at the exam.
